The importance of integrating data between social and health care is unquestionable. Liquidlogic works closely with System C, Graphnet, EMIS Health, Healthcare Gateway, Orion, Cerner and others to provide interfaces to health systems and deliver their benefits.
There are obstacles, like information governance issues, pressing priorities or the lack of expertise, but the process has started and there are examples to show it is not impossible.
One of the projects is the Digital Discharge, which is an interface between social care and hospital systems to aid discharge management. It is already successfully supporting real-time exchange of hospital admission and discharge notifications in Wirral, Lancashire, Rotherham and Hertfordshire Councils. Some of the many benefits:

Shared Care Record allows professionals working in hospitals, community, primary and social care services to have real-time access to the patient information, to communicate and collaborate safely and effectively across disciplines and organisations. It works both ways as social workers can access the patient’s health and GP record, while the healthcare setting are able to view the social care information. Some of the benefits:

The Medical Interoperability Gateway (MIG) implemented in Cumbria County Council is one of the shared care interfaces which helps the collaboration between health and social care teams, improves the communication and helps with time management.
These examples show that the improvement has started, and if it is approached step by step, it will lead to success. The demand is real and urging, so the IT suppliers have to get through the obstacles and provide interfaces to help health and social care teams to their job on the best possible way.
